Whether you are new to chiropractic or just to our office, we are excited to help you. We offer patient centered care that is tailored to your individual needs and goals.
What to Expect on your First Visit
During your first visit, the doctor will take a thorough history of your presenting problem. During this time the doctor will also address any questions or concerns you may have.
Using this information an examination will be performed to provide further insight as to the source of the issue. X-rays may be required by the doctor depending on the nature of the condition. Please tell the doctor if you have had any diagnostic imaging performed or if you may be pregnant.
This information will all be used to determine if you are a good candidate for chiropractic care. If you are, the doctor will discuss a treatment plan with you and care will begin with your consent.

Notice of Privacy Practices (HIPAA)
Please review the Notice of Privacy Practices document before selecting one of the New Patient Packets. This document explains how your personal health information is protected under HIPAA law. If you have questions, we would be happy to answer them when you arrive.

Medicare
Part B Medicare covers %80 of the allowable amount for chiropractic spinal adjustments. It does not cover any initial exam done by the doctor, nor does it cover x-rays or physiotherapy. These services may or may not be covered by your secondary insurance. Please review the Medicare Policy for more information regarding your Medicare Part B coverage.
Insurance
It is important that our patients verify their chiropractic benefits with their insurance company. The Insurance policy page of this packet will guide you through the important questions to ask your insurance company regarding your chiropractic coverage. Please bring this packet along with your insurance Card and ID.
Cash
If we are not filing insurance of any kind, please fill out this new patient packet. If you have any questions, we will be happy to assist you when you arrive.
